Collapsed Jawline in a Male Patient in his 30s
A male patient in his 30s visited with a severe double chin. He visited due to "toad neck" symptoms, where the double chin became severe due to weight gain and the cervicomental angle (jaw-neck angle) disappeared. Upon examination, issues were diagnosed: skin sagging, subcutaneous fat, platysma muscle sagging, and deep fat excess. I mentioned in a previous post that whether one has a weak chin (receding chin) is important for double chin treatment, but his chin tip was buried in flesh, making it hard to diagnose easily. A CT scan was performed, and it was confirmed that correction for a weak chin was not necessary.
Diagnosis and Decision on Necessary Surgery (Highlighted)
The patient wanted a quick recovery due to work issues, so he chose a double chin thread lift instead of an incision facelift. Accordingly, we proceeded with double chin liposuction, double chin muscle tying (+ deep fat removal), and double chin thread lifting. This is an effective and cost-efficient surgical combination for removing double chin fat from a "toad neck."

Dr. Returning Performing Double Chin Liposuction
The patient visited after fasting and wanted surgery on the same day, so we were able to proceed immediately with sedation anesthesia. After injecting sedation (Propofol) and tumescent (subcutaneous anesthesia), the surgery time took as follows:
Time taken to improve toad neck: Only 1 hour
- Double chin & jawline liposuction: 20 mins
- Double chin muscle tying: 20 mins
- Double chin thread lifting: 20 mins
- Total: 1 hour
The reason I explain the surgery time is that the longer the surgery takes, the longer the recovery time and the longer the swelling may last. It is important to perform the surgery as concisely as possible without bleeding. To achieve this, pre-operative diagnosis is crucial, and it is important to perform only the necessary procedures without unnecessary manipulation.
Treatment (Aftercare) for Toad Neck/Double Chin Removal
The treatment is also simple. Unless there is an unexpected event, double chin surgery must be performed without any bleeding. Therefore, we do not use blood bags (drains) or wicks. Usually, you only need to come in for treatment the next day and remove stitches on the 7th day, so there is no burden regarding treatment. Swelling and bruising are not severe, and you can use a compression bandage (face garment) for management purposes for about 1-2 months, just like with a facelift.

Position of Double Chin Thread Lifting
In cases where a face-neck lift is not performed and excess skin cannot be removed, threads must be inserted like this to create a sharp cervicomental angle. Also, it helps achieve skin adhesion while distributing the excess skin evenly so it doesn't pile up at the jaw angle. The threads used here are much thicker and longer strong cog threads than general facial threads. They dissolve over time, but since they create strong adhesion until then, if used well, you can see permanent effects in removing double chin fat from a toad neck.
